Aims for Caboolture Family Network
Caboolture Family Network works within an holistic model, where each family relates to one key staff person who plans, organises and co-ordinates support with the family and/or carer. This option will offer flexible support that will allow the person with the disability, their family and/or carer to design their own support, if they choose.
Caboolture Family Network believes that a person with a disability, their family and/or carer have the same basic rights and responsibilities as any individual. We believe in offering a service that:
· Has a strong and respectful partnership with a person with a disability, their family and/or carer and the wider community;
· Acknowledges that every family and/or carer has a need for ‘time-out';
· Allows a person with a disability, their family and/or carer the opportunities to make choices in their own lives;
· Promotes dignity, respect, privacy, confidentiality and accountability;
· Provides good planning;
· Provides access to all people without discrimination;
· Allows a person with a disability, their family and/or carer to raise concerns without recriminations;
· Assists a person with a disability, their family and/or carer to develop meaningful social and physical inclusion into the local community.
